For health service providers, CIE can reduce costs while simultaneously improving quality and the patient’s experience of care.
Use social determinants data to better understand, predict, and control outcomes for complex patients. We are continuously examining and researching new approaches to enhance access to patient-centered care by promoting integration of our dashboards into day- to-day operations in Federally Qualified Health Centers (FQHCs), EMS/ambulance services and hospitals.
Since many CIE San Diego clients visit community clinics for their healthcare needs, these safety net providers are a critical source of data. When appropriately shared with other health and social service organizations, this information can help to improve client outcomes, speed intake, and reduce duplicative services or follow-ups.
In turn, this knowledge transfer supports capacity building for those community clinics to more effectively care for patients and impacts policy decisions as the CIE San Diego approach begins to document significant cost savings and improvement in outcomes.
