Jennifer’s work has focused on programmatic implementation of Social and Health Information Exchange and the development of innovative initiatives to encourage and incentivize systems improvement within local health care safety net systems. Those projects have included improving access to care, care transitions, integration between behavioral health and primary care, and integration of data systems – all critical pieces of delivering whole person care. Jennifer was a key leader in Alameda County’s Community Health Record and Social Health Information Exchange creation and implementation – from concept phase to full implementation over a 6-year period.
