CIE San Diego

Partners in San Diego’s Community Information Exchange®(CIE) Network are working to improve the service delivery system in our region through an integrated technology platform that coordinates care and shares information electronically to improve the health and well-being of people across San Diego County. The CIE transforms the way social and health services are delivered in our community through a collaborative approach that enables partners to provide holistic, person-centered care. Through the CIE, organizations can embed the infrastructure into their own organizations, leverage the CIE to shape care and prioritize services, and share ownership across the network. The overall goal of the CIE is to advance equity for all.
