211 San Diego/CIE in partnership with Interfaith Community Services, Lifeline Community Services, Neighborhood Healthcare, Multicultural Health Foundation, and Goodwill of San Diego County, offered clients (patients) the opportunity to provide feedback on how their information is viewed, managed and shared within the Partner Community, including how their information is used to support their care coordination.

Justice-Impacted
211 San Diego/CIE sought out to increase our understanding of impact and accessibility, for justice-impacted individuals, upon reentering the community. We partnered with; Neighborhood House Association, Second Chance, Asian Solidarity Collective, Metro Community Ministries, San Diego Workforce Partnership, and the County of San Diego’s: Public Safety Group & Sheriff’s Department.

Food Access

211 San Diego/CIE wanted to understand access challenges and explore connections that work, for individuals who are nutritionally and/or food insecure so, we partnered with agencies and community based organizations who directly support with access. Our partners included; Bridge of Hope SD, Mundo Gardens, Home Start, Olivewood Gardens & Learning Center, Bonita Family Resource Center, Jewish Family Services of SD, American Red Cross (WIC).

Youth & Children
211 San Diego/CIE collaboratively worked with agencies and community based organizations to identify successes, opportunities, challenges for services and resources for Youth, Children, and families. We partnered with; Lived Experiences Inc., Emilio Nares Foundation, San Diego Regional Center, San Diego Center for Children, Somali Family Service of San Diego, and San Diego Youth Services.
