Historically, our health systems and social service delivery infrastructure have not been responsive to or inclusive of the true needs of communities and vulnerable populations, especially people of color. Community Information Exchanges have the potential to promote equity and inclusivity through their community-led model. Drawing from current events, including the COVID-19 pandemic, CIE Summit 2021 sessions will thoughtfully analyze and discuss how to move CIE planning and implementation forward using community-led strategies that enable inclusive and equitable community input and participation.
The Virtual Special Session is an opportunity to learn and network before the Summit. During the one-day session, participants will get an overview of the Community Information Exchange, network with other communities to spur discussion and connections for the upcoming event, and get a sneak peek of the Virtual CIE Summit in September. Featured presentations will highlight the “how to” of an innovative approach, design, strategy implementation, tool, data use, and evaluation of care coordination systems. These sessions will provide an applied focus or strategy that addresses a specific, real-world problem that is impacting individual and community-level health and well-being.
